Abstract:

This publication describes a person-centred, socially inclusive and culturally appropriate enabling approach to provide community care services to frail older people and people with disability. This approach is advocated by IMPACT, a working group formed in NSW and comprising community care representatives from key consumer groups, cared bodies, service provided organisations, industry bodies, the HAA Development Officers Network, government agencies and local government.
