Home Care Package provider activity survey

A survey has been developed to collect totals of service hours and units of care delivered to your home care package clients. This information will assist the Department to understand how best to support you when the Support at Home funding model begins on 1 July 2025. The information collected will not be published and is not an audit of individual provider capacity.

Data may be shared with the Independent Health and Aged Care Pricing Authority (IHACPA) to assist with their Support at Home Costing Study. More information on their costing study can be found here or get in touch with IHACPA using SAHC@ihacpa.gov.au
